The patient-centered medical home (PCMH) is a team-based model of care that transforms comprehensive primary care of children, adolescents, and adults, emphasizing the patients’ wants, needs, and preferences. Led by a personal physician, patients get a full support system and increased education for decision-making and participating in their own care.
